It doesn't sound like she was ever formally diagnosed as gender dysphoric. I can totally see that being a small, very insular town with, unfortunately, a lot of negative attitudes towards gay and transgender individuals. Just be careful to not conflate Destinee's journey, stalled though it may be, with all other trans folk. And none of the obese have been particularly likeable. Those men typically identify as heterosexual. There is no mention of Destinee ever feeling she was female. Her clothing choices weren't always the most flattering but she may not have a lot of resources.
